7/11/2023 EnchantaVaughn Approved 1.Behavior: Upon arrival to the home setting, JaCaeri entered the session in an excited and eager demeanor, evidenced by his need to explain to QMHP-C that he had been following his routine. QMHP-C informed him this session was going to focus on his ISP goals and progress made towards them. 2. Behavior: JaCaeri displayed off task behaviors evident by him starring off at the walls and not engaged in conversation with QMHP-C at times but once he was redirected he was able to gain focus on the session. 1.Intervention: QMHP-C began the session by inquiring about JaCaeri's emotional well-being. QMHP-C asked several probing questions to get JaCaeri to clarify on his actions specifically at home. QMHP-C used active listening and positive feedback to encourage JaCaeri to express how he behaved and explain his actions of disrespect if any at home QMHP-C assisted JaCaeri on brainstorming any situations where he was rude or disrespectful and not following directions. QMHP-C reviewed JaCaeri’s some triggers at home and if he had experienced any symptoms of anger and frustration. QMHP-C assisted JaCaeri in expressing how he was acting and behaving at home, specifically with how he was dealing with his emotions when he became upset. QMHP-C used active listening and supportive statements to encourage JaCaeri, as he shared his feelings. QMHP-C used positive feedback when JaCaeri agreed to participate in the brainstorming activity for the session. 2. Intervention: QMHP-C discussed with JaCaeri some of the triggers he feels make him upset and angry such as following directions, paying attention and completing his hygiene routine. QMHP-C used active listening and supportive statements to support JaCaeri while expressing himself on why he is always bullying his sister. QMHP-C used suggestions to assist JaCaeri with practicing some empathy towards his sister. 1.Response: JaCaeri was receptive to QMHP-C's suggestions for today's session. JaCaeri was able to listen to encouragement and positive feedback from his mother and QMHP-C to follow directions after several prompts and try to listen to what was being said, JaCaeri sighed in a passive aggressive way and did not like having to be told what to do. JaCaeri was able to following directions and accepted most feedback from the QMHP-C. 2. Response: JaCaeri participated and discussed his triggers such as not hearing his answers, evidenced by his ‘I just get so annoyed at people when I don’t get them to do what I want’, comment. JaCaeri stated he just doesn’t’ think before he reacts, evidenced by ‘I just get really upset and angry’, comment. JaCaeri listened to QMHP-C’s suggestions and examples of him being sad or disappointed that he didn’t get what he wanted and how to react.
